How do i make an appointment?

You can call our clinic on 03 90204524 , email or fill an online booking form. Healthcare providers can refer using our medical or allied referral form which can be faxed or emailed.

what should i bring to my appointment?

Please bring your referral from your GP or referring doctor and your medicare card. Having a referral letter helps our healthcare providers understand the concerns your GP or other doctor may have.

Having a valid referral also ensures our patients receive the medicare rebate for their visit.

If you don’t have a valid referral, you cannot claim the medicare rebate, which means higher out of pocket costs.

International students or travellers do not need a referral.
